Distance Services Equipment Request Form

This form is for the use of current HGSE:

• Doctoral students (D3+) living more than 50 miles from Cambridge
• Urban Superintendents Program students who have completed their first year
• Faculty on leave

Use this form to request one of the following equipment kits:

• Digital Voice Recorder Kit, including DS-3300 digital voice recorder, 32 MB xD-picture card, docking station with recharge function, USB cable, and DSS Player Pro software, or
• PC Transcription Kit, including foot switch, stereo headset, DSS player, and transcription software. Note: students must supply 2 AAA alkaline batteries.

The regular loan period is 28 days. Equipment is limited and subject to recall. If there is no demand for equipment you have borrowed, you may renew.
